Student Life Budget Request and Special Circumstance Request
Due March 1 of each fiscal year, existing clubs use this form to secure funds for the upcoming year. For requests outside the normal budget cycle, the same form can be submitted, noting that it is a special circumstance request.
Attendance Form
This form must be completed for all club meetings and events and turned in to Student Life on the minimum of a monthly basis.
Fundraising and Sales Application
This form must be completed and submitted to the Director of Student Life at least 2 weeks before fundraiser begins.
